Barachel Skatepark's two waivers (combined in the one file linked below) must be filled completely and accurately, and submitted in person prior to gaining admittance to the Barachel Skatepark.

Barachel reserves the right to deny admittance to anyone with suspicious or incorrectly completed skatepark forms.

Please note that there is a section to be completed by a parent or guardian of anyone under the age of 18 to be admitted to the skatepark. If that section is not completed and signed in the presence of Barachel staff, then notarization is required.
